fix: honor CLI execution policy flags

Summary

• apply explicit --no-parallel and --parallel overrides without replacing config defaults
• apply --fail-fast to the effective run configuration
• release as 0.4.1 with regression coverage

Verification

• npm run verify (339 unit, 2 integration, lint, build, workflow and package checks)
• reproduced the original news-podcast multi-suite run: 115/115 pass sequentially
