Express, Fastify, Koa, and Hapi errors are captured by their integrations in v11, so the setup*ErrorHandler calls are deprecated. Remove them from the getting started snippets, move Fastify's shouldHandleError to fastifyIntegration, and point Hono to the sentry() middleware option since honoIntegration was removed. Fixes SDK-1482
