BREAKING CHANGE:
- `/v0/create-linked-verifiable-presentation` is replaced by `/v0/add-linked-verifiable-presentations`
- `/v0/remove-linked-verifiable-presentation` is replaced by `/v0/remove-linked-verifiable-presentations`
- In a `ServiceResponse` (`GET /v0/services`, `GET /v0/services/{service_id}`), `presentation_ids: string[]` is replaced by `presentations: LinkedVerifiablePresentation[]`, where each entry holds `presentation_id`, `holder` and `url`
