Fixes all six repository review findings: concurrent inbox deduplication, strict dual-LLM plan contracts, complete replay option hashing, whole-response output limits, whitespace reviewer validation, and corrected memory-decay documentation. Adds focused regression coverage. Verified with a Release build of 105 projects and 449 passing tests.
