Clarifies that applied=false and escalated=false require observe-only behavior even when a fix or prompt patch remains available for human inspection. Documents recognized parser fields without claiming autonomous safety. Documentation and docstrings only; no executable behavior, dependency, version, tag, or package publication changes. Full local suite, lint, types, build, and clean-wheel checks are recorded in docs/plans/healing-result-documentation.md. Hosted CI must pass before merge.
