Add engineering constraints to RDD reviews

Summary

• introduce a flat set of human-approved engineering constraints with explicit scope and effective points
• add a separate RDD engineering check invoked by cold review and completion review
• extend canonical state shapes and focused procedures to carry engineering trace gates

Verification

• all 13 RDD skills pass quick_validate.py
• git diff --check passes
