feat: add agent feedback and session review - #243

Add canonical response and conversation feedback, independent debug and chat collection settings, and administrator feedback review. Exclude empty histories, retain archived review context, and make partial runtime settings updates atomic. Keep feedback storage and review indexes in one consolidated migration, with local schema and ledger reconciliation verified.
