Make publish flow resilient per package (core/cli independent)

Improve release robustness by publishing packages independently and summarizing outcomes.

- split publish into per-package execution
- continue publishing remaining packages when one fails
- emit structured summary of success/failure per package

## Acceptance Criteria
- partial success is visible and actionable
- failed package details include command/log hint
